Terms and Conditions

United Way of the Chattahoochee Valley Volunteer Center Agreement and Release

In connection with my voluntary involvement in activities undertaken for, and with the participation and support of United Way Volunteer Center (UWVC) I hereby agree, for myself, my heirs, assigns, executors and administrators to release and discharge UWCV/HOV, its officers and directors, employees, agents and volunteers (the "UWVC Parties") from all claims, demands, and actions for injuries sustained to my person and/or property as a result of my involvement in such activities, even where caused by the negligence of the UWVC Parties, and I agree to release and hold the UWVC Parties harmless from any cause of action, claim or suit arising there from.   I hereby attest that my attendance and involvement in such activities is voluntary, that I am participating at my own and sole risk.  In addition to the foregoing, I will only participate in UWVC activities and projects that I am physically capable of participation without risk of injury to myself.  Furthermore, to the extent I utilize my own vehicle for transportation or other purposes in connection with a UWVC project or activity, I hereby represent and warrant that I am, and the vehicle is, fully insured to the extent required by law. 

I further irrevocably give the UWVC Parties, its assigns and successors, my consent to use my name, any photograph, likeness, image, voice, videotape, sound recording and biography made of me or in which I may be included in whole or part in any and all media, publications, advertising and publicity in connection with my participation with UWVC and any UWVC activity or project.  Additionally, by signing this document, I release and forever discharge the UWVC Parties from any and all claims or liabilities of any kind which may arise from the use of my likeness.  I acknowledge and agree that I will not receive any compensation in connection with any use of my likeness.

I hereby acknowledge that United Way of the Chattahoochee Valley Volunteer Center is involved in assisting organizations and associations which may from time to time be involved with child care and related matters. I hereby confirm, represent and warrant that I have never been convicted of or charged with a violent crime, or any crime involving or relating to child abuse or neglect, child pornography, child abduction, or any other violent offense, including kidnapping, rape or any sexual offense, nor have I ever been ordered by a court to receive psychiatric or psychological treatment in connection with such crime or crimes.

This release shall inure to the benefit of UWVC and UWVC Parties.  This release shall be governed by the laws of the state of Georgia. I further attest that I have read, understand, and accept the terms and conditions of this release.
